Test results come back normal, and the patient leaves with the assumption that it\u2019s probably anxiety. The appointment ends, but the effects linger.<\/p>\n\n\n\n<p>When clinicians dismiss, downplay or outright ignore a patient\u2019s symptoms \u2014 can result in long-term psychological and behavioral damage. The phenomenon is known in medical literature as \u201csymptom invalidation,\u201d but to many patients, it\u2019s simply \u201cmedical gaslighting.\u201d<\/p>\n\n\n\n<p>Published in Psychological Bulletin, the review synthesized findings from 151 qualitative studies involving more than 11,000 patients. The individuals represented a wide range of contested or difficult-to-diagnose illnesses, including fibromyalgia, long COVID, endometriosis and lupus.<\/p>\n\n\n\n<p>Across the board, the review found that invalidating medical encounters caused \u201ca wide range of negative outcomes,\u201d including shame, suicidality, anxiety and avoidance of future health care.<\/p>\n\n\n\n<p>It is found that patients can question reality. \u201cThey ask, \u2018Am I making this up? Is this all in my head?\u2019 It is also found symptom invalidation is associated with depression, suicidality and health care-related anxiety that actually can rise to the level of trauma responses.<\/p>\n\n\n\n<p>What happens after patients are dismissed<\/p>\n\n\n\n<p>The review categorized the consequences of symptom invalidation into four broad areas:<\/p>\n\n\n\n<p>Emotional harm: Self-doubt, shame, hopelessness.<\/p>\n\n\n\n<p>Health care-related distress: Anxiety, trauma, loss of trust.<\/p>\n\n\n\n<p>Behavioral changes: Avoiding doctors or downplaying symptoms.<\/p>\n\n\n\n<p>Diagnostic delays: These can worsen disease progression and health outcomes.<\/p>\n\n\n\n<p>Patients reported downplaying their symptoms to doctors so they wouldn\u2019t appear dramatic or like they\u2019re exaggerating. Some patients went further, disengaging from the health system altogether.<\/p>\n\n\n\n<p><strong>An epidemic fueled by uncertainty<\/strong><\/p>\n\n\n\n<p>The researchers noted that clinical encounters involving uncertainty \u2014 where there is no clear diagnosis \u2014 are now the fastest-growing type of medical visit. This shift is being driven in part by emerging and ambiguous syndromes like long COVID and postural orthostatic tachycardia syndrome (POTS), which don\u2019t fit easily into standard diagnostic frameworks.<\/p>\n\n\n\n<p>Members of the research team argue that symptom invalidation persists in part because \u201cmedical training hasn\u2019t kept pace with increasing uncertainty.\u201d As they write in the paper, algorithm-based workflows often \u201cleave little room for listening to complex stories that don\u2019t fit neatly into test panels or insurance codes.\u201d<\/p>\n\n\n\n<p><strong>What clinicians can do<\/strong><\/p>\n\n\n\n<p>The experts emphasized that validating a patient\u2019s experience does not require a definitive diagnosis.<\/p>\n\n\n\n<p>It is not recommended reassurance about it \u2018probably being nothing serious\u2019 to patients who have a lot of distress about their symptoms. Patients appreciate clinicians communicating their uncertainty and admitting they don\u2019t know something.<\/p>\n\n\n\n<p>The researchers developed a conceptual model linking symptom invalidation to emotional distress, health care withdrawal and ultimately poorer health outcomes. They argue that reducing invalidation should be a priority for clinician training and systems design.<\/p>\n\n\n\n<p><strong>Patients, pushed into self-advocacy<\/strong><\/p>\n\n\n\n<p>The study also explored what happens when patients are forced to advocate for themselves in response to dismissal \u2014 something many find difficult or unsustainable.<\/p>\n\n\n\n<p>Patients can do research and see if they can find ratings of the clinicians online. But it\u2019s hard for patients to be in a situation like this where they must advocate for themselves so aggressively. One good strategy is to bring a partner, an adult child, a friend or anyone else who can support their statements about symptoms when talking to clinicians.<\/p>\n\n\n\n<p><strong>A call for reflection<\/strong><\/p>\n\n\n\n<p>While the findings draw attention to gaps in clinician training and systemic pressures, Bontempo and her team emphasize that even small changes in how clinicians respond to patients can reduce harm.<\/p>\n\n\n\n<p>The report\u2019s authors conclude that \u201cinvalidation confers a wide range of negative outcomes,\u201d and that these results \u201cmay provide hope to those with difficult-to-diagnose illness that their suffering is real and begins to offer the validation they so desperately seek.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Symptom invalidation can cause emotional harm, healthcare-related distress, behavioral changes, and diagnostic delays, worsening health outcomes.
